Delivery Mode: For the in-person option, the Lifestyle Coach and participants meet together in a classroom or classroom-like setting. For the virtual options, participants can choose either live, interactive group sessions using a phone or videoconference platform (e.g., Zoom) or on-demand (not live) sessions using a Smart phone, tablet, or computer.

MDPP Supplier: Organization offering the Medicare Diabetes Prevention Program (MDPP) for eligible Medicare enrollees.
